5 Lives and Level Select:

During the Character Select screen press B, A, D, C, A, D.  Then Press Start

Level Select:

During the Character Select screen press Down, Up, Up, Down, C, A, B and start.  The Level Select screen will appear after you select your character.

10 Lives and Level Select:

During the Character Select screen press Down, Up, Up, Down, A, B, B, A.  Then press Start.  This code will not give you the real ending.
